Thank you taking Swim Classes at Diver Dan's!
In order to make your registration as smooth as possible, Diver Dan’s provides you with all the paperwork for registration.  Just click on the different forms below, fill them out at your convenience, then bring them into Diver Dan’s!

 

Registration Form
This two page form is needed to register for swim lessons. 

• Please ONLY fill out the Swimmer Information at the top of page one.
• Do NOT fill out the Scheduled Dates of Classes until availability has been confirmed.
• Page two is for Special Needs / Notes.  We can discuss this with up when you come in to register.

Liability Releases
This four page form is needed to register for swim lessons. 

• Page One is the General Liability Release Form.  This is required for everyone of all ages.
• Page Two is the Youth Liability Release Form.  This is required for everyone under the age of 18.
• Page Three is the Emergency Treatment Consent Form.  This is required for everyone under the age of 18.  We request that all parents stay on-site for the entire lesson.  However, we understand that emergencies happen and you may have to leave your child for a short time.  This form is required just in case of an medical emergency.
• Page Four is the "Who can pick up my child" Form.  This is required for everyone under the age of 18.  We request that all parents stay on-site for the entire lesson.  However, we understand that emergencies happen and you may have to leave your child for a short time.  This form is required just in case you are unable to return for an extended period of time.
